Brookside Soccer Club | Liability Waiver

 

This registration is designed to accept the Wilson Management Group, Inc. D.B.A. Teamwork Sports Liability Waiver.  All Brookside Soccer Club rostered players that will participate in Indoor Winter Workouts at Teamwork Sports are encouraged to complete.  Registration should be completed by Parent/Guardian and an electronic signature will be required.
